Change of Address Form
TUSD employees and former employees can use the Change of
Address form to submit address changes to the TUSD Human Resources
department. Submitting this form will ensure that address
changes are processed for TUSD payroll and the Arizona State
Retirement system.
If you are a retiree, please print "RETIREE"
at the top of the form so that Payroll will forward the form
to the Benefits Dept.

ASRS Members: If you are a member of the
ASRS, you must submit the ASRS "Change of Address" form directly
to the ASRS. This form can be located at the TUSD Human Resources
Customer Service office or on the ASRS website at www.azasrs.gov.
