For Office Managers

  • Ensure the Accident Investigation Report (in PDF) is sent to the Workers' Compensation Office via email, fax, or interoffice mail.
  • Ensure employee gives supervisor their Return to Work Status after each doctor visit.
  • Record absences using Sick Leave, with Reason Code OJINJ.
  • If an employee has been placed Off Work by the physician, give the employee paperwork for LOA/Family Medical Leave.
  • Forward all paperwork for LOA/Family Medical Leave requests to the Workers' Compensation Office
